Essential Duties and Responsibilities include the following. Other duties may be assigned.
Evaluates the capabilities and disabilities of consumers to develop goals, objectives and methodologies for consumers in residential facilities. Prepares goals following New Jersey Division of Developmental Disabilities oversight agency and other agency and professional guidelines in accordance with consumers’ treatment team meeting. Discusses and coordinates goals for Individual Habilitation Plan (IHP) with recommendations. Prepares a variety of administrative controls such as goal books, which contain data charts, intervention sheets for use by direct service workers.
Through training and in-service, prepares Direct Service Workers for implementation of goals as outlined in the IHP. Will review goals to
ensure consistency with person centered planning and gentle teaching techniques.
Coordinates program to ensure continuity of care with an outside agency. Participates in individual consumer 30-day review, annual IHP, special Interdisciplinary Team Meeting as needed or required. Writes and presents reports of goal progress, recommending goals or revisions based on data analysis/reports. In conjunction with the Case Manager, coordinates reviews and informs all necessary participants of time and date. Ensures that absent participants receive summary of meeting. Collects assessments from clinical team for presentation of IHP. Obtains signatures, completes checklists and files documentation as needed, required or scheduled. Follows up on goal recommendations, attends outside day program reviews and performs other related functions relative to the experience of each consumer.
Reviews minutes of habilitation plan meetings, ensuring changes in services are documented, that staff is aware of changes and that follow up and modification of schedules and activities are implemented as appropriate. Prepares monthly reports and reviews regularly with supervising psychologist.
Designs and carries out regular staff in-service training (including orientation), maintaining appropriate records of material covered and the participation of the staff members. Provides training to ensure compliance with changing regulations and new or modified skills designed to address specific needs of consumer. Consistent with the principles of person centered planning and gentle teaching.
Performs direct counseling and intervention with consumers, as necessary or advisable. Counsels consumers regarding issues of concern pertinent to their performance at all skill levels. Assists other staff members and residents’ families and advocates in intervention as may be required by circumstance.
Assists in the development and initial project oversight, teaching new managers agency systems, philosophies, and organizational documents.
Monitor existing medical and psychotropic consents to ensure compliance with filing requirements. Prepare psychiatric referrals for review by psychologist. Participates as ongoing member of Human Rights and Incident Review committees.
Participates in emergency or crisis situations as qualified and requested.
Must cooperate and participate fully with Jawonio and/or NJ DDD during an investigation or inspection, including making verbal and written statements as requested by the person(s) doing the investigation.
May assist during consumer behavioral crisis that may occur.
Maintains safe, orderly work area and work practices.
Performs other duties as required.
